Appointment Cancellation Policy:
We understand situations arise in which you cannot make it to your appointment. It is therefore requested if you are not able to make it to your appointment, please provide at least 24 hour notice prior to your visit. This will allow us the opportunity to schedule another patient in that time slot. If you are unable to give 24 hour notice of cancellation, you may be subject to a $50.00 ‘no show’ fee. The fee is the sole responsibility of the patient and must be paid in full prior to the next visit.
Nuclear Testing Cancellation Policy:
We understand situations arise in which you cannot make it to your appointment, however, nuclear testing requires the office to order medical supplies for the testing that cannot be used if you do not make it to your appointment. Therefore, a fee of $250.00 will be charge to the patient to cover the cost of those supplies if you ‘no show’ your appointment without 24 hour notice.
Insurance Change Policy for Physicals
Due to the many policy changes in insurance plans diagnostic testing may be subject to your copay, deductible or coinsurance. While insurances have improved in the coverage of annual preventative care, they no longer consider any testing as preventive care.
Dr. Jasser and the team are committed to giving you the best medical care possible and will continue to do so, despite these changes. Please note, our providers will not change their current standard recommendations for you to have your necessary cardiac testing, bone density screening, or any testing for other chronic medical conditions that are necessary for your healthcare.
Financial Policies
If we accept your insurance, we will file claims on your behalf. All co-payments are due at the time of service unless other arrangements are made. You will receive a statement for deductibles, co-insurance and non-covered services. If you do not have insurance, payment is due at the time of services rendered. Refund Policy
There are no refunds for medical services rendered by Jasser Center for Comprehensive Care or Advanced Nuclear Cardiology. In the event an error is made, our billing department will investigate. If an error or duplicate payment is found, a credit will be issued to your account balance or you may request a check with our office. We will issue a credit within five to seven business days.
Sliding Fee Scale
Jasser Center for Comprehensive Care will offer a Sliding Fee Discount Program to all who are unable to pay for their services. We will base program eligibility on a person’s ability to pay and will not discriminate on the basis of age, gender, race, creed, disability, or national origin. The Federal Poverty Guidelines, http://aspe.hhs.gov/poverty, are used in creating and annually updating the sliding fee schedule to determine eligibility.
